Nursing – Student Reference
If transferring from another nursing school with nursing clinical courses completed you may request to be considered as Advanced Standing, successfully completed nursing courses at another institution. A reference from the Director of the previous school is required.

Advanced Placement Test Form
Students who have participated in the high school Advanced Placement (AP) program may be eligible to receive credit and advanced placement. Official scores must be on file at BHC. For a list of eligible tests, visit Advanced Placement Testing (AP).
FERPA Prevent Disclosure
A student can block release of any and all information. This block is active until revoked by the student.
Academic Suspension Appeal Form
If you have been placed on Academic Suspension and wish to address the appeal committee, please complete and submit per instructions.
Graduation Application 2025-26
Required in order to be considered for your degree or certificate. Please provide one application per degree/certificate you’re seeking. While we are always accepting these, the published deadline ensures that your name will be on all commencement-related publications.
Student Registration Form
The Student Registration Form Enrollment document is used by students to enroll in classes, providing personal information and course selections for the term. It collects details needed to register and process enrollment.
FERPA Authorization to Release
A student can allow BHC to release their protected information to a third party. This release is active until revoked by the student. Third party must mention the document when they inquire. Information may or may not be released immediately.
Change of Major/Program
The Change of Program form allows students to request an official update to their academic program or major. It collects current program information and the desired new program for processing.
Student Change of Name form
The Student Change of Name form is used by students to request an official update to their name in college records. It requires the student’s current and new name along with supporting documentation.
