Steps to receive services and accommodations:
- Contact Disability Services Office.
- Submit disability documentation which determines eligibility and supports request for accommodations.You may complete the following forms and submit them via fax or mail (when signature is required) or e-mail attachment(when a signature is not required) and the Disability Services Coordinator will contact you.Below are important forms you will need:

Verification and Documentation of Disability - Meet with Disability Services staff to develop accommodations plan.
- Introduce yourself to your instructor, deliver the V.I.S.A. forms, and discuss your accommodation needs the first week of classes.

Testing
Extended time, minimum distraction area, use of a reader, scribe, or computer, tape recording answers, and large print.
Note-takers
A fellow student in class may be willing to make a carbon copy of his/her notes. The student may choose to tape the class and later make notes from the tape. Sometimes instructors have a class outline or class notes they will share with the students.
Books on alternative format
Staff members will assist students in obtaining textbooks or other class material from the publisher or Recordings for the Blind and Dyslexic (RFBD), New Jersey. They can also use Kurzweil 3000 to create an audio book if one is not available from the publisher.

Equipment loan
The office has a limited amount of four-track cassette players, cassette tape recorders, handheld magnifiers, and Victor Vibes (RFBD).
Assistive technology
ZoomText, Dragon Naturally Speaking, Jaws for Windows and Kurzweil 3000 software, and CCTVs are available in the Disability Services Office. Contact the Disability Services Coordinator for training. Students may secure their own copy for the home computer when taking online classes by contacting the Office of Rehabilitation Services.
Sign language interpreters
Interpreters are provided for classes and other campus activities.
