Establishing yourself as a client of the Disability Services Office must happen first before support for online learning can be provided. If possible, please make an on-campus appointment with the Coordinator of Disability Services, Susan Sacco. If that is not possible, you may complete the following forms and submit them via fax (when signature is required) or email attachment (when a signature is not required) and the Disability Services Coordinator will contact you.

Note: Use this form to secure verification of your disability if you do not already have signed verification from a professional.

If you already have a relationship established with the Disability Services office, you will want to work with an advisor next to select your classes and then visit with the Disability Services Coordinator about any needed accommodations for the online classes you select. At that time, you may want to request that course materials be converted for you:
